Patient Rights

  1. To be informed of their rights in a clear and understandable manner.
  2. To have their families informed about the care and services available at the center and how to access them.
  3. To receive care that respects privacy and confidentiality.
  4. To be protected from any form of abuse or harm while receiving care.
  5. To receive appropriate protection if the patient is a vulnerable child, a person with disabilities, and/or an elderly patient.
  6. To have all medical and personal information kept confidential and protected from loss or misuse.
  7. To refuse or discontinue treatment.
  8. To request a second medical opinion.
  9. To have personal belongings respected and properly managed.
  10. To be informed of treatment costs, if applicable.
  11. To provide written consent for any non-emergency, surgical, or high-risk medical or therapeutic procedures.
  12. To have general consent and informed consent explained in a clear and understandable manner, with sufficient and accurate information about their medical condition, proposed treatment, and available healthcare alternatives to support informed decision-making.
  13. To receive information about their medical condition, confirmed diagnosis, and approved treatment with the patient’s consent.
  14. To participate in treatment-related decision-making if they wish.
  15. To be informed of any conflicts of interest within the center related to the care provided.
  16. To follow the center’s policies regarding complaints and dispute resolution, and to be informed about how to submit complaints.
  17. To know the names of physicians and healthcare providers involved in their care.
  18. To have access to translation services or resources to overcome language barriers.
  19. To be treated with dignity and respect in accordance with professional standards, regardless of presentation, gender, race, religion, culture, disability, or any other factor.

Patient Responsibilities

  1. To provide all necessary personal and medical information required to deliver appropriate care.
  2. To follow the instructions and recommendations of healthcare providers.
  3. To accept the terms and conditions related to the provided treatment and care.
  4. To provide accurate information regarding their medical treatment.
  5. To accept the consequences of refusing recommended treatment by healthcare providers.
  6. To approve or decline the transfer of care to another physician or healthcare facility when necessary.
  7. To take responsibility for the outcomes resulting from refusal of care or treatment.
  8. To return any borrowed or damaged items to the healthcare facility.
  9. To accept responsibility for payment of any services provided.
  10. To comply with the healthcare center’s rules and regulations, including non-smoking policies.
  11. To inform the medical team of any health conditions that may affect treatment.
  12. To provide feedback to physicians and inform them of any medical concerns encountered.
