- To have their families informed about the care and services available at the center and how to access them.
- To receive care that respects privacy and confidentiality.
- To be protected from any form of abuse or harm while receiving care.
- To receive appropriate protection if the patient is a vulnerable child, a person with disabilities, and/or an elderly patient.
- To have all medical and personal information kept confidential and protected from loss or misuse.
- To refuse or discontinue treatment.
- To request a second medical opinion.
- To have personal belongings respected and properly managed.
- To be informed of treatment costs, if applicable.
- To provide written consent for any non-emergency, surgical, or high-risk medical or therapeutic procedures.
- To have general consent and informed consent explained in a clear and understandable manner, with sufficient and accurate information about their medical condition, proposed treatment, and available healthcare alternatives to support informed decision-making.
- To receive information about their medical condition, confirmed diagnosis, and approved treatment with the patient’s consent.
- To participate in treatment-related decision-making if they wish.
- To be informed of any conflicts of interest within the center related to the care provided.
- To follow the center’s policies regarding complaints and dispute resolution, and to be informed about how to submit complaints.
- To know the names of physicians and healthcare providers involved in their care.
- To have access to translation services or resources to overcome language barriers.
- To be treated with dignity and respect in accordance with professional standards, regardless of presentation, gender, race, religion, culture, disability, or any other factor.
Patient Responsibilities
- To provide all necessary personal and medical information required to deliver appropriate care.
- To follow the instructions and recommendations of healthcare providers.
- To accept the terms and conditions related to the provided treatment and care.
- To provide accurate information regarding their medical treatment.
- To accept the consequences of refusing recommended treatment by healthcare providers.
- To approve or decline the transfer of care to another physician or healthcare facility when necessary.
- To take responsibility for the outcomes resulting from refusal of care or treatment.
- To return any borrowed or damaged items to the healthcare facility.
- To accept responsibility for payment of any services provided.
- To comply with the healthcare center’s rules and regulations, including non-smoking policies.
- To inform the medical team of any health conditions that may affect treatment.
- To provide feedback to physicians and inform them of any medical concerns encountered.
